Kennel Liability & Waiver Agreement

This Liability Waver & Agreement is between River City Veterinary Hospital and Resort and pet owner named

The signing of this Agreement is a one-time requirement and shall remain in full-force and effect, for an unspecified period of time, with its terms applicable to the parties each time Kennel provides services to Owner, to include boarding, daycare, grooming or veterinary care services for Owner’s pet named:

I hereby consent and authorize River City Veterinary Hospital and Resort to receive and board my pet. I understand the resort will use all reasonable precautions for the safekeeping of the described pet, but the resort will not be held responsible in any manner, whatsoever, on account of medical situations that may arise, as it is thoroughly understood that I assume all risks. I also understand the resort personnel are not present continuously after normal business hours.

If an emergency should arise, River City will do their best to contact me, but in the event they cannot, I authorize River City to approve any medical treatment recommended by the veterinarian, including transfer to the closest referral facility. I agree to pay all expenses incurred due to such an event. I understand that in a non-emergency situation, my pet will receive only minimal treatment until I have been contacted for approval.

I understand that River City Veterinary Hospital and Resort follows the AAHA guidelines and my pet must be up-to-date and current on the following: physical and fecal exam, Distemper, Parvovirus, Rabies, and Bordetella. I certify that my pet is not coughing, gagging, sneezing, vomiting, having diarrhea, and is free of all nose and eye drainage associated with infection. I also certify to that I am not aware of exposure with any other pet showing these symptoms in the last two weeks. I understand that River City reserves the right to refuse patients, sometimes, without notice. I understand that River City is not to be held responsible if my pet(s) contract contagious diseases or pests during their stay

I understand that any personal items that are run a risk of being damaged while at the hospital, and River City is not to be held responsible. Any large items that may not fit into our washing machine that become soiled will be returned to me bagged and unwashed.

I understand that it is River City Veterinary Hospital and Resort policy that no two animals shall share a confined space, as there is increased risk associated in stressful situations. As the owner, I understand this risk and, if I elect to have pets boarded together, will under no circumstances hold the hospital or staff liable for any incidents that may occur.

I hereby certify that I have read and fully understand this authorization and assume financial responsibility for all charges incurred to the above pet and agree to pay all such charges at the completion of this stay. I have read and agree to the policies and procedures for River City Veterinary Hospital and Resort and have verified all information listed above.
