Vaccinations:

To prevent the spread of infections diseases, all hospitalized and boarding patients must be current on all vaccines and free of internal and external parasites. The signature below authorizes the doctor to provide these vaccines and/or parasite control. I am the owner of the animal listed above and will be responsible for payment of services rendered.

ALL PROFESSIONAL FEES ARE DUE AT THE TIME SERVICES ARE RENDERED. A deposit may be required for extensive or emergency procedures. I am the owner/agent of the animal listed above and will be responsible for payment of services rendered. I understand that River City Veterinary Hospital does not offer payment plans or billing options, but does accept Care Credit, cash, MasterCard, Visa, American Express, Discover, debit cards, and checks. There will be a fee associated with any returned/bounced checks. Any unpaid balances will be submitted to a collection agency. I am at least 18 years of age.

I am the owner or the agent to the owner of the animal described above, and I have the authority to execute this consent. I hereby consent and authorize River City Veterinary Hospital and Resort to receive, treat, and board my pet. I expressly consent to and authorize River City Veterinary Hospital and Resort, as well as its staff, to hospitalize my pet and to administer vaccinations, medications, tests, surgical procedures, anesthetics and treatments that the Doctors deem necessary for the health, safety and well-being of my pet while it is under their care and supervision. I understand the resort will use all reasonable precautions for the safekeeping of the described pet, but the resort will not be held responsible in any manner, whatsoever, on account of medical situations that may arise, as it is thoroughly understood that I assume all risks. Furthermore, I understand that unforeseeable reactions to treatments are always possible and authorize treatment necessary should any reactions occur. I understand that I am financially responsible for all treatment. I also understand the resort personnel are not present continuously after normal business hours.
