By signing below, you are authorizing our veterinarian and staff to perform initial testing and treatment of your pet, as well as agreeing to pay all the incurred charges. Associated fees with this emergency typically range from $300 - $500, but they could be higher. After initial emergency assessment and treatment has been completed, an itemized estimate will be presented to you for your approval for continuation of care.
Please understand that all charges incurred during this visit must be paid-in-full prior to your pet’s release. There are no payment plans or billing options available. However, you can visit www.carecredit.comto apply for a third-party credit plan.
